Ask Doctor Grover - How often should I have Botox injections?

On average, most patients will notice that they will see a return of ‘dynamic’ wrinkles (those created from muscle contraction) by three months after injections of Botox.

Some patients do not want to see any movement or appearance of any wrinkles and may feel that Botox only lasts them 2-3 months, while other patients may say that their Botox lasted them 6-9 months, but upon examination you see a fair amount of movement.

In general, this is not dependent on age.  You may find someone in their late 20’s or early 30s with very animated wrinkles that are quite apparent with only little muscle contraction, while you may see a 60-year-old patient with few dynamic wrinkles.

Melasma, or the ‘mask of pregnancy’, is discoloration or hyperpigmentation patches usually around the cheeks, nose, and upper lip. It is best treated or improved with cessation of oral contraceptives, intense pulse light lasers, hydroquinone or agents such as Cosmelan. Botox has no effect on melasma. Botox is a neurotoxin that will improve dynamic wrinkles by relaxing or temporarily paralyzing muscles.

Ask Doctor Grover - Rhinoplasty Results are Subjective

A ‘bad nose job’ can be very subjective. A bad nose job can present itself with a undesired cosmetic appearance or an undesired breathing problem. Realize that the nose will go through a lengthy ‘healing’ period following rhinoplasty. This can be anywhere from 1 to 2 years. Do not rush into a rhinoplasty revision; however, it is not unreasonable to discuss your concerns with your plastic surgeon or another if you have significant concerns or anxiety. Avoid undergoing further surgery for at least one year following your initial rhinoplasty.

I prefer to use an external incision on many of my blepharoplasty patients to address the fat (excision or redraping), along with muscle tightening, skin removal and canthal tightening. You may need to have the skin/muscle addressed at this point and should seek further consultation.

Ask Doctor Grover - Blepharoplasty Cost?

There are many different techniques available to perform blepharoplasty. More commonly, upper lid blepharoplasty is performed more similarly from surgeon to surgeon. Lower lid blepharoplasty, will vary tremendously.

Some surgeons will only perform a transconjunctival blepharoplasty through the inner lower lid to remove fat, some will only remove a ‘pinch’ of skin from the outer aspect with or without transconjunctival fat removal or redraping. Some will perform fat contouring and skin/muscle re-draping with an incision just below the eyelashes.

Many options. Costs will vary.

A fractional laser is best suited to rejuvenate the skin, but does not perform a facelift. If your main concern is lax skin and jowls, I would recommend you undergo a facelift. You can achieve some tightening from a fractional laser or other non-ablative laser such as ReFirme or Titan, however, the results are not the same as with a facelift.

Wow!  A facelift in an hour.  I can’t do that.  Please let me know how someone can achieve a beautiful, long lasting, naturally looking facelift in one hour.  Most skilled plastic surgeons will take approximately 2 to 6 hours to perform a lower facelift and then add some more time if you are adding a brow lift and blepharoplasty.  Be careful!
