It is still possible to use the axillary approach. This will be a more likely situation if you have saline implants and are planning on staying with saline implants. I prefer to use an endoscope for my transaxillary breast augmentations.
However, I do not prefer to place silicone implants via the axillary approach. Most often, I will use the periareolar or inframammary approach for primary and secondary cases. Having said this, you should not fear the periareolar approach to your surgery as the scars, many times, heal beautifully.

Ask Doctor Grover - Can Breast Implants Deflate Due to Sleeping Position?
Saline breast implants can fail or deflate at any time following implantation. However, when it happens, it usually becomes apparent following a few days. You can have a partial deflation for a short while; however, it eventually fully deflates if you wait long enough.
Breasts change over time, especially following augmentation. Changes occur with age, weight changes, pregnancy, etc. It is not uncommon to feel that one’s breasts are getting smaller or ‘looser’ as the skin further expands and if you lose weight. This can lead to the feeling of ‘rippling’. You should seek an appointment with your board certified plastic surgeon to evaluate your breasts. You may benefit from a silicone gel implant.

Ask Doctor Grover - Breast Implants Settling - How Long?
It takes 4-6 weeks for your breast implants to ’settle’. There are many factors involved, including the tightness of your breast skin, envelope, the size and profile of your implant, the surface of the implant, and whether your implants were placed entirely under the muscle or partially under the muscle.
In reality, it is not that the implants have to ‘move’ down or ’settle’, it’s just that the lower pole of the breast tissue and skin needs to expand. Be patient.

Ask Doctor Grover - Tips for Finding the Best Plastic Surgeons
Prepare yourself prior to going in for a consultation. You will want to ask yourself many questions before going in for a consultation and upon your search for your plastic surgeon. Three questions to start with include:
(1) Is your surgeon certified by The American Board of Plastic Surgery (go to ABMS.org to find out);
(2) Is your surgeon a member of ASPS (plasticsurgery.org) and ASAPS (surgery.org), and;

Ask Doctor Grover - How do I know if I have Capsular Contracture?
It is likely you may have a capsular contracture if you feel that your breasts are quite hard and do not appear as though they have ’settled’. The other possibility is that your saline implants were overfilled significantly more than is recommended. This would also cause your implants to feel quite hard.

Ask Doctor Grover - Replacing Breast Implants Increases Risk of Capsular Contracture?
Replacing your implant with a smaller one in the same pocket should be fine if using a gel implant. If you are thin with little breast tissue, you will want to avoid a saline implant in the subglandular position to avoid rippling. With regards to capsular contracture, you should be fine keeping your implants in the same pocket if you currently have soft breasts.
Realize, however, that anyone with silicone implants can at some point develop a capsular contracture. There is evidence to show that the submuscular position has a decreased incidence of capsular contracture but this may not be your best option at this point and given that you are a personal trainer.

Ask Doctor Grover - How do I find the best Plastic Surgeons?
It can be very confusing on how to select a surgeon for their cosmetic surgery procedure. I believe it is best to select a surgeon who is certified by The American Board of Plastic Surgery ( www.abplsurg.org), who is a member of The American Society of Plastic Surgeons (www.plasticsurgery.org), and someone who is a member of The American Society for Aesthetic Plastic Surgery (www.surgery.org).
Next, I would visit the state medical board to see that their license is current without any limitations. Following this, I would go to the surgeon’s personal website to look at credentials, testimonials, before and after photos etc.
Finally, I would recommend you go visit the surgeon and see if you are confidant in and comfortable with the surgeon. Determine if you feel the staff is helpful and friendly. See if you can speak with previous patients.

Ask Doctor Grover - How do I choose the right breast implant size for me?
Many factors are involved in deciding the best size for a given patient. First of all there is no one size that only fits a patient. I prefer to use a number of factors in deciding which size is best. First of all, I like to know if my patient is trying to achieve a conservative B cup or a large DD size. Body proportions, chest dimensions, breast dimensions, amount of breast tissue present, ‘looseness’ of the skin etc are all important factors. Have faith in your Board-Certified plastic surgeon. Ask Doctor Grover - Is Dual Pane the best technique for Breast Implants Surgery?
I prefer to use a dual plane, partial retropectoral technique for most of my breast augmentation patients because, in my hands, I feel it offers the nicest, most natural results. I feel it offers benefits of both the submuscular technique and subglandular technique.
