Ask Doctor Grover – Nipple Reduction Surgery for Women and Men

Nipple reduction is not an uncommon request. Although I perform many more breast procedures on females, it is interesting that I find that more males request ‘nipple reduction’. This procedure is often performed differently for males and females. I find that most males prefer their nipples to be as small or least protruding as possible. This procedure is performed under local anesthetic in about one-half an hour.

Botox and Juvederm used together create amazing results

Fillers such as Juvederm or Restylane are best to plump lips as they are fillers.  Botox, on the other hand, effects the nerve impulses to muscles that create lines or wrinkles (dynamic rhytids).  Having said this, I have on occasion injected Botox around the lips for “smoker’s lines” and patients have felt that it also helped them achieve a fuller lip by effecting the way the lip roll is.

Ask Doctor Grover – What is the appropriate amount of Botox?

Many patients benefit from Botox for treatment of dynamic wrinkles of the forehead, glabella (11′s), crow’s feet, and droopy brows. Fifty units of Botox is a very average amount to begin with. Some patients may require more on their second treatment.

Ask Doctor Grover – Will I get used to my butt implants?

Most of my buttock augmentation patients feel they are part of them. As with breast implants, patients tend to feel that implants are incorporated into their body and self image and do not think about them. Some of the buttock implants used now are softer than previously used. It also helps if you have a decent amount of padding or fatty tissue to cover the implants.

Methods for Buttock Augmentation

Two methods for buttock augmentation include:

1) Fat grafting using your own fat cells; and

2) Buttock augmentation using silicone implants

Patients with adequate donor fatty tissue elsewhere on the body may be a candidate for fat transfers. However, many patients who desire buttock augmentation do not have much excess fatty tissue and must rely on a silicone implant to achieve an augmented or fuller buttocks.

Ask Doctor Grover – Eyelid Surgery Performed With Local Anesthesia

Eyelid surgery can be performed without general anesthesia.  It is reasonable to undergo upper eyelid blepharoplasty under straight local anesthesia.  One can also perform some types of lower lid blepharoplasty procedures under local anesthesia as well.  For some more aggressive procedures, patients will benefit from some sort of sedation or general anesthesia.

Wearing Contact Lenses After Eyelid Surgery?

Many patients can wear contact lenses after 3 or 4 weeks following upper and lower blepharoplasty.  Some patients, however, may have some dry eyes or chemosis following blepharoplasty and should wait until this seems to be resolved.  Care should be taken if a canthopexy or canthoplasty of the lower eyelid was performed as this may disrupt or effect the result.

When finding a plastic surgeon, I would recommend you search for one who is certified by The American Board of Plastic Surgeons.  You can find board-certified plastic surgeons in your community by searching www.plasticsurgery.org or www.surgery.org.  I am proud to be a member of The American College of Surgeons thus I will put FACS behind my name, however, you may find some surgeons who are not  plastic surgeons that are a member of The American College of Surgeons.  They may be certified in other specialties instead of plastic surgery.  It does get confusing.

Ask Doctor Grover – What’s the difference between gynecomastia and liposuction?

Gynecomastia surgery may involve liposuction and/or excision. Many patients who present with concerns of gynecomastia or enlarged male breasts benefit from some form of liposuction. I prefer power assisted liposuction, but ultrasonic liposuction is another good option. Some patients, however, have some very firm, glandular tissue that will require excision with an incision made on the underside of the areola.

Gynecomastia procedure – Incision Location

Patients suffering from gynecomastia or male breast enlargement can be treated with several different surgical modalities.  These include:

  • Liposuction only (preferably power-assisted liposuction)
  • Liposuction along with surgical excision
  • Excision of the glandular tissue (subcutaneous mastectomy) only.

Surgical excision can be performed using semicircular incisions along the lower border of the areola or small ‘stab’ incisions along the areola.  The appropriate method chosen will depend on the underlying problem.
