Orange County Cosmetic Surgeon Q & A

Ask Doctor Grover – Does Botox cause Melasma?

Melasma, or the ‘mask of pregnancy’, is discoloration or hyperpigmentation patches usually around the cheeks, nose, and upper lip. It is best treated or improved with cessation of oral contraceptives, intense pulse light lasers, hydroquinone or agents such as Cosmelan. Botox has no effect on melasma. Botox is a neurotoxin that will improve dynamic wrinkles by relaxing or temporarily paralyzing muscles.

Ask Doctor Grover – Is there such a thing as an Hour Long Facelift

Wow!  A facelift in an hour.  I can’t do that.  Please let me know how someone can achieve a beautiful, long lasting, naturally looking facelift in one hour.  Most skilled plastic surgeons will take approximately 2 to 6 hours to perform a lower facelift and then add some more time if you are adding a brow lift and blepharoplasty.  Be careful!

Ask Doctor Grover – Non-surgical Face Lift is Botox, Juvederm, Restylane

A non surgical Face Lift generally refers to fillers, Botox, and certain laser procedures. These treatments are ideal alone for patients who do not require a Face Lift. Face Lifts will help address excess lax skin and underlying tissues. Some patients who undergo Face Lifts will also benefit from these treatments such as Juvederm, Restylane, and Botox.

Ask Doctor Grover – How fast will I see the results of dermal fillers?

The wonderful thing about many dermal fillers today is that you will see immediate results.  Juvederm is a hyaluronic acid or sugar that will attract water.

Because of this, you will see some additional swelling or fullness usually within 2-3 days.  This usually resolves within the first week following injection.  Rare patients may develop significant swelling, but is less likely.

Ask Doctor Grover – Botox or dermal fillers?

Botox is wonderful for minimizing dynamic wrinkles that occur with muscle contraction.  If you don’t have the wrinkles while at rest and not contracting any muscles, then it likely will work well.  There are instances when someone needs both Botox and a dermal filler, especially in the glabellar area between the eyebrows (the ’11′ sign) when these have become deep grooves.

Ask Doctor Grover – Performing Face Lift while effects of Botox still present?

Botox and Fillers are complimentary to many Face Lifts. Each procedure can improve signs of aging at different levels and regions. It is usually not ideal to perform all of these procedures in the same setting however as the effects of the filler and botox will be less precise.

Ask Doctor Grover – Placement of sutures makes all the difference in Face Lift Surgery

The number of sutures placed in any procedure is not important.  What is important is that  the appropriate number of sutures are placed in the right place, using appropriate techniques to achieve the desired result.

Mini Facelift (short-scar) vs. Full Facelift

A mini-facelift or short-scar facelift is an appropriate facelift for the patient with minimal signs of facial aging, minimal skin laxity, minimal jowling and minimal looseness in the neck.

A full facelift is more appropriate for patients with greater signs of aging including greater facial skin laxity, worse jowling and more neck skin excess and looseness.  The scars should be minimally noticeable with either technique as long as too much tension is not placed on the skin.
