Most breast reductions will lift the breasts as well. There are rare instances when a breast reduction will be performed with liposcuction alone. Liposuction would rarely ‘lift’ the breast unless there is fantastic elasticity. Many breast reduction techniques will utilize the same incisions used for a breast lift with or without a breast implant.

There are many options when considering breast augmentation…silicone, saline; smooth, textured; round, anatomical; moderate profile, high profile; on top of the muscle, completely under the muscle, or partially under the muscle; lift, no lift etc.
Generally speaking, if a patient has a somewhat droopy or ptotic breast and would like to avoid additional scars for a breast lift, they will be better served by having their implants placed partially under the muscle (dual plane) or on top of the muscle. Having said this, patients who have lost weight or have had their breast tissue atrophy following nursing will have a much more naturally feeling breast with silicone implants versus saline implants. As you can see, there are many options.

Can implant exchange increase capsular contracture risk?
February 17th, 2009I have had implants for 7.5 years with no problems that I am aware of! However, they are def larger than is proportional with my frame as my rib cage is very small and they make me a 32DD when I was a small B before. I would ideally like them to be a C- cup and to maybe sit a tiny bit lower and am therefore considering having smaller implants put in.
Would you advise keeping the implants in the same position or moving placement? They are currently placed above the muscle as I am a personal trainer and aerobics instructor and didn’t want any of the muscle jumping etc… associated with under the muscle placement.
However I am mainly concerned with keeping the likelihood of capsular contraction down to a minimum- is using the original pocket for a new smaller implant going to increase the chances of this occurring do you think? Thanks in advance!
Replacing your implant with a smaller one in the same pocket should be fine if using a gel implant. If you are thin with little breast tissue, you will want to avoid a saline implant in the subglandular position to avoid rippling. With regards to capsular contracture, you should be fine keeping your implants in the same pocket if you currently have soft breasts.
Realize, however, that anyone with silicone implants can at some point develop a capsular contracture. There is evidence to show that the submuscular position has a decreased incidence of capsular contractures but this may not be your best option at this point and given that you are a personal trainer.

Do implants move, shift or change shape after pregnancy?
February 17th, 2009I had breast augmentation, under the muscle, using saline smooth shelled implants 8 years ago. I had a child three years ago and my implants are now visible through the skin with rippling and the left implant seems to have shifted slightly causing mild aching. Does it sound like these need to be replaced or is this all normal?
A woman’s breast will change following pregnancy and nursing and time alone. Following pregnancy and nursing, breast will enlarge and engorge, and then will atrophy or involute. This will leave the breast ‘droopier’ and thinner, and can cause the nipple to descend. If you are thin to start with and have little breast tissue, you will notice that you can feel the implant much more. This is especially common with saline implants where rippling can be noticed. You should visit your board-certified plastic surgeon to have him/her evaluate if you would benefit from an implant exchange to a high profile silicone implant with or without a lift.

I’m 50 – am I too old for breast implants?
I’m turning 50 this year, still in good shape but feeling gravity all over! Can I still have breast implant surgery and what would reasonable expectations be for a woman my age?
If you are asking, you probably are not too old.
There is no age limit to breast augmentation or cosmetic surgery for that matter. If you are healthy and have the desire, then you are a good candidate. Breast augmentation with silicone or saline implants is one of the most requested cosmetic surgery procedures today. It is anticipated that many more women of all ages (over 18) will also undergo a breast augmentation. You should do some further research on the internet and select a few plastic surgeons to visit for a consultation. A good place to start is surgery.org. Newport Beach Breast Implants – Smaller Revision?
February 11th, 2009Getting smaller implants – 700cc to 500cc feasible?
Hi. Four years ago I made the stupid choice of getting 700cc (Round smooth silicone, high profile, 14.4 x 6.2cm)breast implants. I am a tall girl, 5′ 81/2″ and 135 lbs. I wear a 36 band bra. My current cup size is WAY to large! My breasts look extremely fake and i am very self-conscious. Before the 700cc implants I had 400 cc anatomical implants to correct tubular A cups that were also asymetrical. I felt the 400ccs were too small. The 400cc (12.5 cm x 13 cm x 4.9cm) implants were submuscular and looked very natural, they were just small on my broad frame and seemed to be 10 miles apart. I got the 700cc implants to get a cleavage. I hate the high profile, they look really fake and I wanted a natural look. I am getting a revision soon and the doctor recommended Mentor Moderate plus profile, size 450 or 500cc smooth round silicone implants. The dimension of the 450cc implants is 13.6 x 4.2, the 500cc is 14.1 x 4.3cm. I would like a small D, right now I wear a 36D but I know it’s the wrong size because the bra cups fit on the breasts like a baseball cap. I just want breasts that look “real”. He said he would have to do a lift (hopefully just around the areola). Do you think going from 700cc to 450 or 500cc is feasible? Can this still produce pretty breasts that look “real”? What is your experience with downsizing?
It is always possible to downsize your implants, but it may not always be the right thing. In your case, you mention that you previously had 400cc anatomical breast implants but was not satisfied with the size. 450 or 500 cc moderate plus implants will not be much bigger than the 400s you had before.
Ask your surgeon to place several temporary sizers during surgery to help you achieve a nice balance and result you are seeking.

3 most important questions to ask a breast surgeon?
I’ve almost personally made the final decision for breast implants. Just beginning search for the right plastic surgeon. I am worried I’ll freeze up during consultations and forget to ask really important questions.
Give me a cheat sheet of 3 must-ask questions for the surgeons? Very very appreciated! -JJ
Prepare yourself prior to going in for a consultation. You will want to ask yourself many questions before going in for a consultation and upon your search for your plastic surgeon. Three questions to start with include:
(1) Is your surgeon certified by The American Board of Plastic Surgery (go to American Board Of Plastic Surgery Website to find out);
(2) Is your surgeon a member of ASPS (plasticsurgery.org) and ASAPS (surgery.org), and;
(3) Does my surgeon have a valid license to practice medicine/any actions taken against my surgeon (check your state’s medical board’s website). Good luck.
