Areolar reduction is a nice procedure for the right patient with the right type of breasts. Most breast reductions and breast lifts are performed along with an areolar reduction as part of the procedure. Areolar reduction, by itself, is requested on occasion, and does also produce a tightening and lifting effect to ones breasts.
An isolated areolar reduction can be performed under local anesthesia, IV sedation, or general anesthesia. It will take most surgeons between one or two hours. It is not appropriate for some patients and usually is best after childbearing is complete.

Ask Doctor Grover – Reducing Breast Implants Size: How Long Should I Wait?
You can change your implants at any time; however, this may not be the wisest thing to do early on. Generally speaking, you should avoid re-operation in the first 3 months unless there is an emergency that is ‘forcing’ you to do so. Your breast will evolve over the first 3-4 months and you will start getting ‘used to’ your new breasts. You likely had a detailed conversation with your plastic surgeon prior to undergoing this procedure and jointly decided on the size you were going for. Correct?
Give it some time. You may be very glad you did not rush into something too hastily. It does not have to do with bleeding.

Ask Doctor Grover – Increased Capsular Contracture Risk with Breast Implants Replacement?
Replacing your implant with a smaller one in the same pocket should be fine if using a gel implant. If you are thin with little breast tissue, you will want to avoid a saline implant in the subglandular position to avoid rippling. With regards to capsular contracture, you should be fine keeping your implants in the same pocket if you currently have soft breasts.
Realize, however, that anyone with silicone implants can at some point develop a capsular contracture. There is evidence to show that the submuscular position has a decreased incidence of capsular contractures but this may not be your best option at this point and given that you are a personal trainer.

Most breast reductions will lift the breasts as well. There are rare instances when a breast reduction will be performed with liposcuction alone. Liposuction would rarely ‘lift’ the breast unless there is fantastic elasticity. Many breast reduction techniques will utilize the same incisions used for a breast lift with or without a breast implant.

You should give it some time before rushing in for another surgery. Of course, you can have another procedure and change your implant to a smaller size, but your body is still accommodating to having your implants placed. You may feel that you actually like your size in a few months. Although an implant exchange can be relatively ‘easy’, any surgery entails some risks and costs, of course. The other thing to consider is that the difference between a 500cc and 475cc high profile implant is not that significant.

Yes, you can add more saline to your current implants; however, it is generally not advised. First of all, each saline implant has a certain fill range that is recommended for each sized implant. If you exceed the recommended volume by too much it will increase your chances for scalloping along the edges and will make your implant and breast much harder.
Additionally, you may void your warranty on your implants as you are potentially damaging the one-way valve.
Realize, surgery is not without potential risk, so consider your options seriously. Another thing to consider is which technique was used to place your implant. If it was placed by the periareolar or inframammary approach, one could fill it up more, but if the implant was placed via the transaxillary (underarm) or transumbilical approach, you will damage the implant.

Can implant exchange increase capsular contracture risk?
February 17th, 2009I have had implants for 7.5 years with no problems that I am aware of! However, they are def larger than is proportional with my frame as my rib cage is very small and they make me a 32DD when I was a small B before. I would ideally like them to be a C- cup and to maybe sit a tiny bit lower and am therefore considering having smaller implants put in.
Would you advise keeping the implants in the same position or moving placement? They are currently placed above the muscle as I am a personal trainer and aerobics instructor and didn’t want any of the muscle jumping etc… associated with under the muscle placement.
However I am mainly concerned with keeping the likelihood of capsular contraction down to a minimum- is using the original pocket for a new smaller implant going to increase the chances of this occurring do you think? Thanks in advance!

Do implants move, shift or change shape after pregnancy?
February 17th, 2009I had breast augmentation, under the muscle, using saline smooth shelled implants 8 years ago. I had a child three years ago and my implants are now visible through the skin with rippling and the left implant seems to have shifted slightly causing mild aching. Does it sound like these need to be replaced or is this all normal?
A woman’s breast will change following pregnancy and nursing and time alone. Following pregnancy and nursing, breast will enlarge and engorge, and then will atrophy or involute. This will leave the breast ‘droopier’ and thinner, and can cause the nipple to descend. If you are thin to start with and have little breast tissue, you will notice that you can feel the implant much more. This is especially common with saline implants where rippling can be noticed. You should visit your board-certified plastic surgeon to have him/her evaluate if you would benefit from an implant exchange to a high profile silicone implant with or without a lift.

I’m 50 – am I too old for breast implants?
I’m turning 50 this year, still in good shape but feeling gravity all over! Can I still have breast implant surgery and what would reasonable expectations be for a woman my age?
If you are asking, you probably are not too old.
There is no age limit to breast augmentation or cosmetic surgery for that matter. If you are healthy and have the desire, then you are a good candidate. Breast augmentation with silicone or saline implants is one of the most requested cosmetic surgery procedures today. It is anticipated that many more women of all ages (over 18) will also undergo a breast augmentation. You should do some further research on the internet and select a few plastic surgeons to visit for a consultation. A good place to start is surgery.org. This is the website for The American Society for Aesthetic Plastic Surgeons (ASAPS). These are board-certified plastic surgeons who specialize in cosmetic surgery. Good luck.

Could my Breast Implants be Encapsulated?
I’ve had saline implants for a year now. I kept telling my doctor that it felt as if the left one had never even “set in” and that both feels unnatural. He brushed this off. I have an appointment next week…Now they are saying it is probably encap. Does this sound right? They are both so hard I now turn my body to give a hug! The right side is somewhat softer, but nowhere near others I have talked to.
It is likely you may have a capsular contracture if you feel that your breasts are quite hard and do not appear as though they have ‘settled’. The other possiblity is that your saline implants were overfilled significantly more than is recommended. This would also cause your implants to feel quite hard.
