Ask Doctor Grover – Under or Over Muscle for Breast Implants Surgery?

In general, I prefer to perform most of my primary breast augmentation procedures partially under the muscle using a dual plane technique.  This offers several benefits including nice superior pole slope, lower capsular contracture rates and an overall natural appearance.  There are instances when one has implants in the subglandular position and may have significant rippling with saline implants or capsular contracture and they would benefit from a pocket conversion.

Ask Doctor Grover – When should I replace my silicone breast implants?

There is no set rule as to when you should change your implants. Your implants, whether they are Allergan (McGhan) or Mentor (J&J) are warranteed for your lifetime in case of deflation or rupture. You should try not to stress or change your lifestyle to try to avoid implant failure.

Ask Doctor Grover – Can Breast Implants be placed through Tummy Tuck Incision?

Yes, it is possible to place your implants via the abdominoplasty approach. This, however, is not the best idea as you have limited visibility and will be disrupting a portion of your inframammary fold and potentially affecting the shape of your result.  One smaller, but potentially disasterous complication could occur if you were to develop an infection from your abdominoplasty that would now be in direct communication with your new foreign body/implants. Additionally, the scars from the periareolar or inframammary approaches can heal beautifully.

Ask Doctor Grover – How much do Silicone Breast Implants Cost?

The cost for breast augmentation will vary across the country and even vary within each city. The total cost of the procedure will include the surgeon’s fees, the facility fees, the anesthesia fees, the implant fees and perhaps the lab fees. Each situation is different.  Some surgeons may operate in their own office based operating room and others may do the surgery in a free standing surgery center. Some may use a nurse anesthetist while others an anesthesiologist. So many variables. Anyway, gererally speaking, the cost of silicone implants are about $1000 more than the cost of saline implants.

Liposuction is an excellent way to achieve body contouring for patients. The ideal patient has mild to moderate excess fatty tissue with excellent skin elasticity. Many patients following childbirth may not be ideally suited to liposuction alone of the abdomen as they may have excess, lax skin with separation of the abdominal muscles.

Ask Doctor Grover – Is cellulite caused by liposuction?

Liposuction does not cause cellulite, nor does it treat it.  In some instances, liposuction could make cellulite appear a bit worse as it may leave the skin looking a bit more ‘loose’ if there is poor elasticity.  Newer technologies, such as VelaShape are FDA approved devices using radiofrequency, deep suction massage and infrared laser to help improve the appearance of cellulite.

One can achieve ‘six pack’ abs on their own if they are able to get their body fat content low enough. Some patients, however, require some assistance with liposuction. Etching can be performed, but can also leave unacceptable depressions and scarring.
